Privacy Policy

Management of Personal Health Information

​

The practice has strict guidelines on confidentiality, patient consent, and consent for a 3rd party to obtain information, transferring of health information and access and security of personal health information.

It adheres to the National Privacy Principles Act and the Health Records and Information Act.

​

Urgent Results

​

Your doctor or the practice nurse will attempt to contact you immediately via the contact details supplied.
If our attempts at contact are unsuccessful, a letter will be forwarded to your address to contact us urgently for an appointment.


It is very important that you always update your new address/contact details with the practice in case of a change of address or change of
contact details.

Non-Urgent Results

​
We will contact you via SMS to advise that you need to make a non-urgent appointment to see your doctor.

 

It is your responsibility to book a follow up appointment with your doctor to discuss the test results, after any test is being done.

​

We will send 3 reminders for non-urgent results, if you will not book an appointment for result discussion, based on the type of the

test and after review with the treating doctor we may send you the last recall in form of a Registered Post letter or choose no further action.

Please note: if your results are normal, we will not contact you.


You are welcome to phone us to check if your results have been returned to us, but due to patient privacy and confidentiality laws, results will not be discussed over the phone. Results can only be discussed during a face-to-face or a Telehealth consult, booked with your doctor.

Did Not Attend (DNA) and Cancellation Policy

​
If you can no longer attend your appointment, please cancel or reschedule through our booking app or ring the practice.

​Please note a fee of $40 will apply if you cancel less than 2 hours prior to your appointment or fail to attend your appointment.

No further appointments will be offered until this fee is paid.

​Non-attendance of an appointment means that another patient is missing out on seeing the doctor.

After Hours

​
For after-hours emergency medical attention, please attend the nearest Hospital Emergency Department .
The Closest Emergency Department to our practice is : Fiona Stanley Emergency Department, 11 Robin Warren Dr , Murdoch WA 6150

Recalls and Reminders

​
Patients are contacted for recalls and reminders usually by SMS or Phone.
If our attempts at contact are unsuccessful a letter will be forwarded to your registered address to contact us for an appointment.

Confidential information is not included in these letters. All personal information is stored on a program specific, encrypted computer data base.
​

Use of AI Dictation Software's

​

Our doctors may use Dictation software installed on their computers, or they may use online AI dictation apps/websites to assist them with preparing the consult notes. The online AI based apps/websites will listen to the doctor and the patient during the consult through microphone and after the end of the consult it will generate the consult note. Your voice will not be saved on any device and will be destroyed by the AI app/website after the consult note is generated. We will ask for your consent for use of such apps/software/websites in your new patient form and at time of booking an appointment. If you do not want your doctor to use the dictation assistant, you need to advise your doctor at the beginning of your consult.

Email

Electronic Communication is available via email. Please note that our email is not monitored 24/7.

Strictly no clinical information or results, or clinical advice will be given over email.

​

Please do not disclose any personal information/Health conditions in an email.

​

We will never ask you for your payment details or a payment, through email. If you receive an email

asking you to pay any money, please contact our practice and advise us. 

​

All personal emails are stored on a program specific encrypted computer data base system.
​

Transfer of Records:

 

Transfer of records to the new practice of your choice will incur a fee, payable by you before the transfer happens.

The fee is to cover admin and transfer costs.

The practice will do it`s best to process transfer within 2 weeks.

For confidentiality reasons, medical records will be transferred securely.

If you decide against paying the transfer fee, a health summary will be sent to your practice of choice.

 

​Prescription Policy:

​

We WILL not prescribe any schedule 4 or 8 medication or drugs of addiction for new patients. You need to arrange transfer of records AND you need to have prior related specialist assessment and report with clear advice for the type of medication and recommended dosage. You need to know that even a specialist advise may not guarantee the prescription of certain medication and prescription is completely at the discretion of our doctors.

Abusive Behavior Policy:

 

Our doctors and staff have the right to be always treated with respect. We have ZERO tolerance for threatening/violent/abusive behaviors (including verbal) toward our doctors and staff. In such situations offenders will be asked to leave the premises immediately (even if they have an appointment with a doctor on that day) and if required WA Police will be called to attend the situation, and offenders will be removed from our patient list.

Infection Control Policy:

​

Infection control prevention and control is very important goal in our general practice. Our goal is to make it a safe environment for patients, doctors and staff.

​

Patients need to contact the reception and inform the reception prior to come in if they have any potential infectious symptoms (included but not limited to sore throat,cough,fever) . Your appointment may change to a telehealth consult in such situation. If you are attending the general practice in person you need to wear a mask to help with prevention of spreading the infection.

​

We highly recommend only the patient comes to the practice for the consult and in case of a minor, only the child with one parent come to the practice. Avoid coming with all children/partner, if possible, as it may place them at unencessary risk of infection.  

 

COVID infection is still common in the community. Patients with fever, sore throat, cough , runny nose, body ache, diarrhea symptoms are required to do a RAT test at home before they come to the practice .

If the RAT test is positive, please DO NOT come to the practice and call reception to change your consult to telehealth option.

We do not provide RAT tests at the general practice. You can purchase your RAT test from the Southlands Pharmacy next door to us.

​

Billing Policy:​

​

We are a mixed billing general practice. During the week we will bulk bill consults for children under 16 years of age and also for patients over 65 years who hold a valid pension/concession card. Any other age group with or without a Pension card will need to pay the consult fee and then receive their rebate from Medicare Australia. We will assist you to process your rebate.

​

After hours / Weekend consults will be privately billed for all patients from all age groups.

​

Your payment for consultation Fee will incur a surcharge Fee processed directly by the EFTPOS provider.

Continuous Education

The practice is committed to supporting the doctors, nurses and administration staff to participate in on-going education.

Practice Security/Safety:

We Keep no cash on premises. We Keep no Schedule 8 medicine on premises.

We have CCTV cameras inside the practice which are monitoring the premises 24/7 for patient/staff/doctors' safety. Only the Southlands General Practice authorized personnel will have access to the CCTV footage. Southlands General practice hold all the rights to use the footage recorded by the CCTV cameras for the WA police, court or any other organization which it will be necessary to provide video evidence.

Disclosure:

 

We may disclose your personal information to third-party service providers who assist us in delivering our services, such as IT providers and billing companies. We ensure that these providers adhere to privacy standards comparable to ours.
